Dear Parent:
I am writing to seek your permission for showcasing your son's
/ daughter's classroom participation, accomplishments, artwork
and
creative writing on my personal website (http://mr-leb.com)
The 7th GRADE NOTEBOOK. I believe that this is a unique educational
opportunity for my students.
The goal of my website is to help develop students' pride and self-esteem,
increase their classroom participation, encourage their creativity
through art and creative writing and involve parents and relatives
in student education and accomplishments.
Yours truly,
Richard Lebsock
7th Grade Teacher
PARENTS' PERMISSION

by listing projects, writing and art under the student's first name
only - last names are not revealed. The student will not be required
to relinquish his/her copyright to any story or drawing published
- students continue to own the copyright to his/her art and creative
writing.
Please indicate your wishes by checking the following:
__ My child's work can be published on the Internet
__ Photographs of my child participating in classroom or school
activities can be published.
__ My child's work may be published, but not his/her photograph.
__ I would prefer that my child's work and picture not be published
